Data Subject Application Form to the Data Controller

1. General Information

In accordance with Article 11 of the Law on the Protection of Personal Data No. 6698 (“LPPD”), individuals whose personal data are processed (“Applicant”), defined as the data subject in the LPPD, are granted the right to make certain requests regarding the processing of their personal data.

This application form has been prepared to identify your relationship with Reform Spor Sistemleri Sanayi Ve Ticaret Anonim Şirketi (“Reform Spor Sistemleri”) and, if any, to determine your personal data processed by Reform Spor Sistemleri in full and to enable a correct and timely response to your request. In order to ensure the security of your personal data and to prevent unlawful data transfers, Reform Spor Sistemleri may request additional information to verify identity and authorization. The responsibility lies with the Applicant if the information provided is not accurate and/or up-to-date or if the requests are unauthorized.

Pursuant to Article 7 of the Communiqué on the Principles and Procedures for the Application to the Data Controller, if the Applicant’s request is answered in writing, no fee will be charged for up to ten pages. A processing fee of 1 Turkish Lira may be charged for each page exceeding ten pages. If the response to the application is provided in a recording medium such as CD or flash memory, the cost of the recording medium may be requested.

2. Scope of the Right to Apply under Article 11 of the LPPD

The Applicant may apply to Reform Spor Sistemleri and request the following:

  • To learn whether their personal data are processed,
  • If personal data have been processed, to request information about such processing,
  • To learn the purpose of processing personal data and whether they are used appropriately for their purpose,
  • To learn the third parties to whom personal data are transferred domestically or abroad,
  • If personal data are processed incompletely or inaccurately, to request correction and to request notification of such correction to third parties to whom the personal data have been transferred,
  • To request the deletion, destruction, or anonymization of personal data in case the reasons for their processing have ceased to exist, even if they were processed in accordance with the LPPD and other related laws, and to request notification of such operation to third parties to whom the personal data have been transferred,
  • To object to the occurrence of a result against the person by analyzing the processed data exclusively through automated systems,
  • To request compensation for damages in case of suffering damage due to the unlawful processing of personal data.

3. Application Method

Pursuant to the first paragraph of Article 13 of the LPPD, applications regarding these rights must be submitted to us in writing and signed or through other methods determined by the Personal Data Protection Board (“Board”).

In this context, written applications can be submitted by printing out this form and delivering it through:

  • In-person application by the Applicant,
  • Via notary public,
  • Through Registered Electronic Mail (KEP) address, secure electronic signature, or mobile signature,
  • Via e-mail sent from the e-mail address previously notified to us by the Applicant and recorded in our system.

Reform Sports Systems may request additional information to verify the identity of the Applicant in case of applications made via e-mail and may take necessary measures.

Your applications submitted to us will be responded to in writing or electronically within thirty days from the date of notification or receipt, depending on the nature of your request, in accordance with Article 13, Paragraph 2 of the PDPL.
